- The application form should be in prescribed Form (‘A’ or ‘B’ or ‘C’) in complete shape as prescribed in Meghalaya Civil Service (GPF) Rules, 1985 duly sealed and signed by the Head of Office/Department.
- GPF Nomination, if any, and Death Certificate from the appropriate authority to be enclosed along with Form ‘C’ in respect of death case.
- List of surviving family members of the deceased subscriber stating age and relationship.
- Guardianship Certificate in case the claimant is a minor.
- Succession Certificate if the claimant is not a family member.
- In case of a Gazetted officer, 3 (three) copies of specimen signature to be enclosed.
- Date of retirement to be mentioned in the GPF final application.
- Whether there are certificate for Temporary withdrawals/Non-refundable advances withdrawn during last 12(twelve) months preceding the date after retirement/death with T.V. No. and date.
- Last fund deduction to be mentioned in the final payment application.
- Date of Birth to be mentioned in the application in case of death case.
- Name of the Treasury Office from where payment of GPF final payment is desired.
